Galway bay fm newsroom -
The Chairman of the Regional Health Forum West is calling for staff to stand down following the report from the Health Information and Quality Authority into the death of Savita Halappanavar at UHG.
The 31 year old died on October 28th last, seven days after her admission to a gynaecology ward
HIQA identified that there was a general lack of provision of basic fundamental care, for example not following up on blood tests.
It concludes that through a review of Savita's healthcare record, a number of missed opportunities which, had they been identified and acted upon, may have potentially changed the outcome of her care.
Councillor Padraig Conneely wants a meeting on the issue to he held at Merlin Park in the next fortnight
